OK, this is what I was going to come back with:

Today, most lawyers (I believe) are members of the American Bar Association.  I checked their website.  They refer complaints to the state - and to the address, basically, that I already gave you.  However, here is some info -- names, fax numbers, etc., that might get you started:

Directory of Lawyer Disciplinary Agencies (webpage):

It gives these two addresses with phone/faxes:
